The Michiana Soccer Association
annually distributes several publications to our players, their parents,
Coaches, and Team Parents. We also make copies of these publications
available through this web site. Please feel free to download and
print copies of the publications that you find here. However, keep
in mind that these materials are protected by copyright to the Michiana
Soccer Association (© 2007). Please do not alter or misrepresent
these materials in any way.
The publications are stored using
the Adobe Acrobat "pdf" file format. Adobe Acrobat Reader

Medical Release Form
|
All players must submit
a Medical Release Form to their coaches at the first practice meeting
of the season. Without a copy of this form, signed by their parent/guardian,
a player may not participate in MSA practices or games. |
